Zusammenfassung

Dünndarmdivertikel werden in echte und nichtechte, sog. Pseudodivertikel, unterschieden. Mit Ausnahme des Meckel-Divertikels, welches als echtes Divertikel im terminalen Ileum lokalisiert ist, handelt es sich bei den übrigen Dünndarmdivertikeln um Pseudodivertikel. Bei diesen besteht die Divertikelwand nur aus Mukosa, Submukosa und gelegentlich Serosa. Sie treten an der Eintrittsstelle der Mesenterialvenen durch und befinden sich meist auf der mesenterialen Seite des Dünndarmes. Im Unterschied dazu ist beim Meckel-Divertikel die Darmwand vollständig ausgestülpt. Symptomatische Divertikel sind selten und fallen durch Blutung, Obstruktion, Invagination oder Divertikulitis auf. Etwa die Hälfte aller Dünndarmdivertikel wird zufällig entdeckt. Einer Resektion bedürfen nur symptomatische Divertikel oder Meckel-Divertikel mit bestimmten Risikofaktoren (erhöhtes Alter, männliches Geschlecht und ektopes Gewebe). Zufällig in der Diagnostik entdeckte Divertikel sollten nicht reseziert werden.
