Erreger können entweder direkt oder indirekt über das Auslösen einer Autoimmunreaktion das Rückenmark schädigen. Der Begriff Myelopathie beschreibt eine Schädigung des Rückenmarkes, während die Myelitis eine Entzündung des Rückenmarks im engeren Sinn darstellt. Dabei werden unterschiedliche Begriffe entsprechend der Lokalisation der Schädigung im Rückenmark verwendet, die auch mit unterschiedlichen klinischen Präsentationen einhergehen: Eine transverse Myelitis ist in der Regel auf ein oder wenige spinale Segmente beschränkt, während eine longitudinale extensive transverse Myelitis (LETM) sich über drei oder mehr Segmente ausdehnt. Auch spinale Kompressionen können z. B. durch Abszesse auftreten. Der schwerpunktmäßige Befall der Vorderhornzellen wird als akute schlaffe Paralyse (acute flaccid paralysis, AFP) bezeichnet.
