Zusammenfassung
Das venöse Kreislaufsystem enthält ca. 80 % des gesamten Blutvolumens. Die zartwandigen Venen sind stark kollateralisiert und stehen unter niedrigem Blutdruck. Die zentripedale Transportrichtung zum Herzen ist durch Venenrückschlagklappen gesichert. Diese Merkmale bedingen Erkrankungen vom thrombotischen und postthrombotischen Formenkreis. Die akute tiefe Venenthrombose tritt an Beinen und Armen auf, kann zentrale Venenabschnitte erfassen und Lungenembolien auslösen. Während Armvenenthrombosen eine gute Prognose haben, sind tiefe Beinvenenthrombosen durch Rezidive und postthrombotische Syndrome gefährdet. Konservative Therapien stehen heute im Vordergrund der Behandlungspfade. Lysetherapien sowie offen chirurgische oder endovaskuläre Verfahren sind bei größeren Venen zur Entfernung der Thrombuslast diskutabel. Permanente Kavafilter haben sich nicht durchsetzen können. Bei chronischen venösen Verschlusssyndromen stehen operative und endovaskuläre Rekonstruktionsmethoden zur Verfügung. Allerdings sind die Ergebnisse beider Methoden trotz adjuvanter Hilfsmittel, z. B. AV-Fistel, Venenklappenplastiken, nicht befriedigend.
