Zusammenfassung

Die iliakale Endofibrose ist eine Sonderform der peripheren arteriellen Verschlusskrankheit, die zum ersten Mal im Jahre 1985 bei Radrennfahrern entdeckt und näher beschrieben wurde. Es handelt sich hierbei um eine am ehesten durch die repetitive mechanische Belastung verursachte Intimafibrose der A. iliaca in unmittelbarer Nähe zur Iliakalgabel mit resultierender Lumeneinengung. Die Symptome sind meist belastungsabhängige Oberschenkelschmerzen, die die Karriere der oft ambitionierten Athleten gefährden. Ein unauffälliger Untersuchungsbefund mit tastbaren Fußpulsen unter Ruhebedingungen erklärt die oft jahrelange verzögerte Diagnosestellung und eine mutmaßlich sehr hohe Dunkelziffer. Verdächtig ist ein Abfall des Knöchel-Arm-Dopplerindex nach Belastung auf < 0,66 (Normalwert > 0,9), woraufhin eine weiterführende Diagnostik (oft MR-Angiographie) veranlaaat werden sollte.
