Prophylaxe venöser Thromboembolien in der nicht-operativen Medizin

Zusammenfassung

Patienten mit akuten internistischen Erkrankungen haben ein erhöhtes venöses Thromboembolierisiko, dessen Ausmaß wie in der operativen Medizin von der Kombination expositioneller und dispositioneller Risikofaktoren bestimmt wird. Drei placebokontrollierte Studien führten zur Zulassung verschiedener niedermolekularer Heparine und von Fondaparinux zur Prophylaxe bei Patienten mit akuten internistischen Erkrankungen. Im Unterschied zu manchen orthopädischen Patientengruppen, die keiner operativen Therapie bedürfen, werden diese Substanzen bei internistischen Patienten generell in Hochrisikoprophylaxe-Dosis verabreicht. Niedermolekulare Heparine haben auch ihren Stellenwert zur venösen Thromboembolieprophylaxe in der Neurologie, Onkologie, Intensivmedizin und im ambulanten Versorgungsbereich. Bei Blutungsneigung, Niereninsuffizienz oder unsicherer Resorption kann unfraktioniertes Heparin intravenös in laborkontrollierter Dosierung appliziert werden. Bei Kontraindikation gegen eine medikamentöse Prophylaxe sollten physikalische Maßnahmen, bevorzugt die intermittierende pneumatische Kompression, eingesetzt werden.
