Zusammenfassung
Das Gefäßsystem entsteht ab der 3. Embryonalwoche. Hierbei auftretende Entwicklungsstörungen können persistierende Gefäßanomalien zur Folge haben. Dies betrifft besonders herznahe Arterien (doppelter Aortenbogen, rechtsliegender Aortenbogen, Isthmusstenose, A. lusaria). Persistierende Arterien können an Nieren und Extremitäten (A. ischiadica, A. mediana) auftreten. Die Hämodynamik der Makrozirkulation wird mit physikalischen Modellen beschrieben. Für den Druckabfall an Gefäßstenosen ist der Übergang von laminarer in turbulente Strömung bei Steigerung der Fließgeschwindigkeit von übergeordneter funktioneller Bedeutung. Die Pulswellengeschwindigkeit bildet Alterungsvorgänge der Makrogefäße gut ab. Die Organdurchblutung wird besonders über den peripheren Widerstand durch Regulation der Gefäßweite in der Mikrozirkulation instantan eingestellt. Die schnelle Gefäßtonusregulation erfolgt über sympathische Gefäßnerven, wobei jedoch zahlreiche weitere Faktoren auf lokaler und systemischer Ebene Einfluss nehmen. Autoregulative Anpassung des Arteriolentonus kann Zunahmen des Widerstandes proximaler Gefäßsegmente (z. B. bei arteriellen Stenosen) bis zu einem Abfall des poststenotischen Blutdrucks auf etwa 60 mmHg kompensieren. Das Gefäßendothel trägt umfangreich zu homöostatischen Funktionen in der Gefäßwand und des Gerinnungssystems bei. Der physiologischen Endothelfunktion kommt eine wichtige präventive Rolle für degenerative Gefäßveränderungen zu.
