Zusammenfassung
Seit Mitte der 90er Jahre stützt sich die operative Versorgung der Leistenhernie vornehmlich auf die Verstärkung der Leistenregion durch Implantation von die Bruchlücke weit überlappenden, großflächigen, textilen Netzen oder alternativ durch direkten Verschluss der Bruchlücken mittels Kunststoff-Plugs. Während bei den flachen Netzen bereits die Integration in Narbe oder in lokales Gewebe die Fixation des idealerweise spannungsfrei eingebrachten Implantates gewährleistet, erfordert der Plug-Stopfen eine suffiziente Narbenfixation in der Bruchlücke zur Prävention einer Migration bei lokaler mechanischer Belastung. Angesichts der gestörten Narbenbildung mit Ausbildung minderwertigen Kollagens bei vielen Hernienpatienten erscheint es nicht überraschend, dass im Vergleich zu flachen Netzen der Plug-Verschluss mit erhöhten Rezidivraten bzw. Migrationsraten behaftet ist.
