Zusammenfassung

Epigastrische Hernien gehören zu den primärenventralen Hernien und entstehen nicht als Folge einer vorausgegangenen Inzision. In diesem Kapitel wird die Pathophysiologie geschildert, und die chirurgischen Verfahren zur Versorgung epigastrischer Hernien werden diskutiert. Während die Direktnaht nur noch bei sehr kleinen Befunden vertreten wird, haben Netzreparationen an Bedeutung gewonnen. Netze werden oft in den präperitonealen Raum („underlay“) oder in IPOM-Position implantiert. Die retrorektale (Um Missverständnissen vorzubeugen, werden in diesem Buch die englischen Bezeichnungen der Schichten verdeutscht bzw. übernommen und kursiv gedruckt.) Netzimplantation erfordert ein größeres chirurgisches Zugangstrauma und findet eher bei grossen, komplexen Hernien Anwendung. Die Ergebnisse der offenen Reparation mit Netz sind vielversprechend. Abschließend wird in diesem Kapitel die Problematik der Rektusdiastase dargestellt, und es werden Reparationstechniken gemeinsam mit der jeweiligen Abdominoplastik diskutiert.

    Jean-Baptiste-François Léveillé (1769–1829): Studierte am Hôtel-Dieu in Paris unter Pierre-Joseph Desault (1744–1795) – bevor dieser nach Berlin an die Charité ging – und wechselte einige Jahre später nach Pavia (Italien), wo er sich mit Antonio Scarpa (1752–1832) befreundete. 1800 kehrte er nach Paris zurück und widmete sich der Behandlung der Armen. Lévéllier war wissenschaftlich sehr aktiv und veröffentlichte verschiedene Arbeiten auch mit Scarpa zusammen.
