Zusammenfassung

Das Komplikationsspektrum beim refraktiven Linsenaustausch (RLA) ist weitestgehend identisch mit dem der Kataraktchirurgie mit Intraokularlinsen (IOL-)​Implantation. Bei Implantation einer phaken IOL (pIOL) ist das Komplikationsspektrum etwas anders, da bei diesem Eingriff die eigene natürliche Linse erhalten bleibt. Sowohl dem RLA als auch der pIOL-Implantation gemein sind Komplikationen, die auf die Anästhesietechnik und die Eröffnung des Augeninneren per se zurückzuführen sind.

Mit zunehmender Verwendung von der topischen, subtenon- oder Parabulbäranästhesie anstelle der Retrobulbäranästhesie werden heute Komplikationen aufgrund der Anästhesietechnik wie retrobulbäre Blutung (Abb. 21.1), Bulbuspenetration, allergische Reaktion auf das Anästhetikum und Kreislaufbeeinträchtigung bis hin zur Atemlähmung bei akzidenteller retrobulbärer Injektion in den Sehnerven sehr selten beobachtet.
