Zusammenfassung
Die offene Nierenchirurgie hat heute ihren Stellenwert immer dann, wenn endoskopische, minimal-invasive Techniken nicht verfügbar sind oder – selten – ein endoskopisch nicht lösbares, operatives Problem eine Konversion zum offenen Operieren erfordert. Die Hauptindikation ist der Nierentumor, entweder als Nierenteilresektion oder als Nephrektomie. Als Zugangswege werden retroperitoneal der Flankenschnitt (Komplikation: Relaxation der Bauchwand, Pleura- und Rippenverletzung) oder transperitoneal der Rippenbogenrandschnitt, selten auch der Pararektalschnitt (Komplikationen: Verletzung des Darms, Pankreas, Milz), genutzt. Die Nierenteilresektion weist spezifische Komplikationen auf (R1-Resektion des Tumors, Nachblutung, Urinfistel, Funktionseinbuße der Niere durch Ischämie). Präoperativ erhobene Nephrometrie-Scores (RENAL-Score, PADUA-Score) können die Wahrscheinlichkeit zu erwartender Komplikationen vorhersagen. Die Tumornephrektomie wird in Abhängigkeit vom T-Stadium des Tumors am häufigsten durch eine Nachblutung kompliziert.
