Zusammenfassung
Bei Morbus Scheuermann handelt es sich um Wachstumsstörung der Wirbelsäule mit Verschmälerung der Bandscheiben, Keilwirbelbildung, Deckplatteneinbrüchen und Kyphose im betroffenen Bereich. Die Krankheit kann thorakal, thorakolumbal oder lumbal auftreten. Mechanische, genetische, haltungsbedingte und eventuell auch psychologische Faktoren spielen bei der Ätiologie eine Rolle. Pathogenetisch liegt eine Verminderung der mechanischen Festigkeit des Knorpels des Apophysenrings während des pubertären Wachstumsschubes zugrunde. Durch den hohen Innendruck des Nucleus pulposus der Bandscheiben kommt es zum Durchbrechen von Bandscheibengewebe durch die knorpelige Apophyse in den Wirbelkörper. Im thorakalen Bereich liegt eine zunehmende, fixierte Kyphose und im lumbalen Bereich liegen eher Schmerzen und Flachrücken vor. Typische Veränderungen im Röntgenbild sind Schmorl-Knötchen, Randleistenhernien, Keilwirbel und Bandscheibenverschmälerungen. Langzeitstudien zeigen, dass Patienten mit Morbus Scheuermann ein erhöhtes Risiko für Rückenschmerzen haben und dass die Kyphose auch im Erwachsenenalter weiter zunimmt. Bis zu einem Gesamtkyphosewinkel von 50° ist in der Regel keine Therapie notwendig, bei Fixation oder schwerer Haltungsinsuffizienz eventuell Physiotherapie. Ab einer Kyphose von 50° und erhaltenem Wachstumspotenzial erfolgt Korsettbehandlung. Ab einer Kyphose von 70° ist eventuell eine Operation indiziert.
