Zusammenfassung
Das epiphrenische Divertikel ist eine sehr seltene Erkrankung, die auch in der Ära der minimal-invasiven Ösophaguschirurgie eine chirurgische Herausforderung geblieben ist. Die meisten Patienten sind asymptomatisch und bedürfen keiner Behandlung. Es sind geschätzt allenfalls ein Drittel der Patienten, bei denen die Symptomatik (in der Regel Dysphagie, Regurgitation, Aspiration) schwer genug ist, für eine operative Behandlung in Betracht zu kommen. Der erfahrene US-amerikanische Speiseröhrenchirurg Mark Orringer hat dazu treffend gesagt: „Eine meisterhafte Inaktivität bei asymptomatischen oder wenig störenden Divertikeln ist eine gute Praxis, auch wenn in unserer Zeit der minimal-invasiven Chirurgie und Stapler eine Divertikulektomie eine lohnende Trophäe für einen hyperaktiven Chirurgen ist.“ Der Indikationsstellung kommt eine besondere Bedeutung zu, sowohl für den erhofften Erfolg als auch und gerade für schlechte postoperative Verläufe.
