Neonatale Alloimmunthrombozytopenie und weitere fetomaternale Inkompatibilitäten

Zusammenfassung

Analog zur Pathogenese der Rhesusinkompatibilität kommt es durch den Übertritt von mütterlichen Antikörpern – meist Antikörper der Klasse Immunglobulin G gegen das paternal vererbte thrombozytäre Antigen HPA-1a (human platelet antigen) – zu einer Destruktion der kindlichen Thrombozyten. Darüber hinaus ist heute eine zunehmende Zahl weiterer thrombozytärer Antigene, z. B. HPA-3a, HPA-5b, als Auslöser spezifischer antithrombozytärer Antikörper beschrieben. Die durch Antikörper vermittelte Thrombozytendestruktion kann bei der Alloimmunthrombopenie schon in utero ablaufen und intrakranielle Blutungen ab dem 2. Trimenon zur Folge haben.

Der Autor Prof. Gortner ist im Dezember 2019 verstorben.
