Idiopathische entzündliche Myopathien

Zusammenfassung

Die juvenilen idiopathischen entzündlichen Myopathien sind eine heterogene Gruppe systemischer Autoimmunerkrankungen, bei denen muskuläre Schwäche und entzündliche Veränderungen der Skelettmuskulatur im Vordergrund stehen. Am weitaus häufigsten sind die juvenile Dermatomyositis, die Overlap-Myositis sowie die Polymyositis. Die Einschlusskörpermyositis ist die häufigste Myositis des älteren Patienten, im Kindesalter tritt sie nur sporadisch auf. Ebenfalls sehr seltene Entitäten sind die granulomatöse und eosinophile Myositis, die Makrophagen-Myofaszitis, die fokale und orbitale Myositis. Die nekrotisierende autoimmune Myopathie nimmt häufig einen schweren chronischen Verlauf. Neben diagnoseweisenden Laborparametern wie Myositis-spezifischen und Myositis-assoziierten Antikörpern wird eine Reihe von Differenzialdiagnosen diskutiert.
