Zusammenfassung

Akute respiratorische Erkrankungen im Kindesalter sind ganz überwiegend durch Viren ausgelöst. Winterliche Epidemien sind die Regel. Die Krankheitszeichen sind initial sehr ähnlich, es gibt nur wenige spezifische Anfangssymptome, z. B. (Pseudo-)Krupp-Symptomatik durch Parainfluenza-Viren. Für die Pathogenese ist die hauptsächlich infizierte Zellpopulation, die Stärke der Immunreaktion sowie das Alter und die Veranlagung des Kindes ausschlaggebend, letzteres insbesondere hinsichtlich einer Asthmareaktion. Die Diagnostik erfolgt in der Regel durch (Multiplex-)PCR. Antigen-Schnellteste für einige Erreger sind ebenfalls verfügbar. Die Behandlung ist supportiv – nur wenige Maßnahmen sind allerdings hinreichend evidenzbasiert. Antivirale Therapien sind wenige verfügbar. Antibiotikagaben sind unwirksam zur Komplikationsprophylaxe. Für die Influenza-Prophylaxe gibt es eine aktive Impfung, für die passive RSV-Prophylaxe Indikationen bei drohenden schwerwiegenden Verläufen im Säuglingsalter.
