Zusammenfassung
Infektionen der unteren Atemwege sind häufig und stehen gemeinsam mit der COPD auf dem 3. Platz der weltweiten Todesursachenstatistik. Von thoraxchirurgischer Relevanz sind Infektionen entweder, wenn sie trotz optimaler medikamentöser und supportiver Therapie voranschreiten, oder aber aufgrund lokaler Komplikationen einer dringenden chirurgischen Intervention bedürfen. Man unterscheidet unspezifische Infektionen, wie Lungenabszesse, Bronchiektasen und ihre Folgeerscheinungen sowie spezifische Infektionen, seien sie bakterieller, fungaler oder parasitärer Genese. Die Chirurgie der Tuberkulose hat in ihrer Bedeutung in den letzten Jahrzehnten immer mehr abgenommen, jedoch bestehen auch heute wieder Operationsindikationen in der operativen Therapie der multiresistenten Tuberkulose sowie bei Folgeerscheinungen nach narbig ausgeheilter Tuberkulose. Die häufigste thoraxchirurgisch relevante Pilzerkrankung ist die Aspergillose bzw. das Aspergillom. Anatomische Resektionen sowohl bei der Tuberkulose als auch bei Aspergillomen sind technisch anspruchsvoll. Seltenere entzündlich-infektiöse Lungenerkrankungen von chirurgischer Relevanz sind die Aktinomykose, welche sowohl bildgebend als auch klinisch einem pulmonalen Malignom ähnelt. Operationsindikationen bestehen hier nur zur Diagnostik sowie Eröffnung von Abszessen. Die kausale Therapie ist konservativ medikamentös. Die pulmonale Echinokokkose ist eine parasitäre Erkrankung mit zystischen Lungen- und Leberläsionen, wobei der Mensch als Fehlwirt befallen wird. Die operative Therapie ist die einzig kurative Therapie.
