Zusammenfassung
Eine 64-jährige Frau wird aus der psychiatrischen Klinik, wo sie wegen eines „progredienten depressiven Stupors“ behandelt worden war, wegen unklarer Bewusstseinsstörungen übernommen. Eine CT war unauffällig gewesen.Klinisch finden sich bei der soporösen Patientin eine generalisierte Tonuserhöhung, gesteigerte Muskeleigenreflexe und irreguläre Myoklonien im Gesicht und an den Extremitäten. Routinelabor, Bildgebung und Liquorbefund (Eiweißerhöhung auf 123 mg/dl) waren nicht wegweisend. Die Patienten stand wegen Hypothyreose unter einer Substitution mit Schilddrüdenhormonen (T3, T4, TSH waren unauffällig). Es fanden sich Thyreoperoxidase-Antikörper (TPO-AK) von 397U/mn (Norm: <34 U/ml) und Thyreoglobulin-AK von 660 U/ml (Norm <115 U/ml). Damit war die Verdachtsdiagnose einer steroidresponsiven Enzephalopathie bei Autoimmunthyreoiditis (SREAT) zu stellen (früher: Hashimoto-Enzephalopathie). Nach 5-tägiger Steroid-Pulstherapie (1 g/d) war die Patientin wieder voll orientiert. Die SREAT ist eine seltene Erkrankung, bei der es strittig ist, inwieweit es sich um eine eigene Entität handelt oder eine „unspezifische Assoziation der Hashimoto-Thyreoiditis mit einer Autoimmun-Enzephalitis“.
