Zusammenfassung

Die Anatomie des Hiatus oesophageus ist bedeutend für die Funktion des ösophagogastralen Übergangs, als Komponente des unteren Ösophagussphinkters. Bei der Hiatushernie ist der erweiterte Hiatus oesophageus die Bruchpforte. Hiatushernienchirurgie ist somit immer auch funktionelle Ösophaguschirurgie. Eine obligate Operationsindikation besteht bei den Hernientypen II, III und IV. Entscheidend ist die akribische mediastinale Adhäsiolyse mit dem Ziel, eine ausreichende Länge abdominellen Ösophagus wieder spannungsfrei in das Abdomen zu reponieren. Bei den Typ-I-Hernien hingegen ist eine Operationsindikation nur im Zusammenhang mit der Antirefluxbehandlung – nach Versagen der konservativen Therapie und eindeutiger funktioneller Abklärung (Manometrie, Refluxmessung) – zu stellen.
