Zusammenfassung
Die myasthenen Syndrome sind gekennzeichnet durch eine Blockade der neuromuskulären Transmission durch Autoantikörper, seltener gegen andere Endplattenproteine wie MuSK, Titin und LPR4. Pathophysiologisch resultiert hieraus meist eine morphologische oder funktionelle Zerstörung der Endplatten. Klinisch kennzeichnet sich die Myasthenie durch Muskelermüdbarkeit, fluktuierende Doppelbilder bis zur Generalisierung. Therapeutisch kann bei der rein okulären Myasthenie die symptomatische Behandlung mit Cholinesterasehemmern ausreichend sein. Vor allem bei generalisierter Myasthenie ist eine langfristige Immunsuppression erforderlich. Diese kann mit initial mit Corticosteroid erfolgen, erfordert langfristig jedoch meist den Einsatz von Azathioprin, Ciclosporin A oder IVIG. Bei therapierefraktären oder krisenhaften Verläufen kommen auch Plasmaphereseverfahren, Eculizumab oder Rituximab zum Einsatz.
