Zusammenfassung

Arthrodesen des unteren Sprunggelenkes sind indiziert bei schmerzhaften degenerativen, entzündlichen oder posttraumatischen Gelenkerkrankungen, kontrakten Fehlstellungen (z. B. Koalition) oder nicht beherrschbaren Instabilitäten. Eine detaillierte präoperative Diagnostik ist unverzichtbar für eine befundangemessene Planung des Eingriffs. Dabei ist zu klären, welche Gelenke des unteren Sprunggelenkes versteift werden müssen und welche Begleiteingriffe (Osteotomien, Sehnentransfers) erforderlich sind. Trotz standardisierter Operationstechnik und stabilen internen Osteosyntheseverfahren ist in der Nachbehandlung eine mehrwöchige Teilbelastung des Fußes erforderlich. Die Rückfußarthrodesen sind zudem komplikationsbehaftete Eingriffe (Wundheilungsstörung, Pseudarthrose, Fehlstellung). Maßgeblich für den Behandlungserfolg ist die achsengerechte Reorientierung des Rückfußes, während die röntgenologisch offensichtlichen Anschlussarthrosen oft ohne klinische Relevanz bleiben.
