Zusammenfassung
Die Arthrodese des oberen Sprunggelenkes (OSG) ist ein sicheres Verfahren zur Behandlung der fortgeschrittenen, konservativ nicht mehr ausreichend beeinflussbaren Arthrose des OSG. Zwar wird die Bewegungsfunktion gegenüber den konkurrierenden Verfahren, der OSG-Endoprothese und den periartikulären Achskorrekturen dabei geopfert; vorteilhaft ist aber die sehr gute Belastbarkeit der Versteifung bei körperlich beanspruchenden Tätigkeiten, die Möglichkeit zur Defektrekonstruktion, die Anwendbarkeit bei Bandinstabilitäten und die sehr gute Vorhersagbarkeit der Beschwerdereduzierung. Vorteile für eine Versorgung mittels Endoprothetik konnten bisher nicht belegt werden. Die Osteosynthese erfolgt typischerweise mittels großvolumigen Osteosyntheseschrauben, winkelstabilen Formplatten oder bei zusätzlicher Pathologie des USG mittels retrogradem Arthrodesenagel.
