Zusammenfassung
Chronische Krankheiten und Mehrfacherkrankungen (Multimorbidität) bestimmen die aktuellen Anforderungen im hausärztlichen Kontext. Neue, diagnoseübergreifende Prinzipien für diese Patienten werden im Chronic Care Modell beschrieben, u. a. Selbstsorge, Koordination, evidenzbasierte Entscheidungshilfen und Informationstechnologie. Sowohl die Verhaltenstherapie als auch die Allgemeinmedizin sehen in der Arzt-Patienten-Kommunikation das Schlüsselelement ihres Handelns. Die Psychoedukation, das strukturierte Problemlösen, die kognitive Umstrukturierung sowie das einfache Case-Management haben sich in Hausarztpraxen vielfach bewährt. Insgesamt sind Patienten mit psychischen Erkrankungen wie Depressionen, Angststörungen, posttraumatischen sowie anderen Belastungsstörungen und somatoformen Störungen in Hausarztpraxen sehr häufig. Die niedrigschwellige Hausarztpraxis ist eine zuverlässige – und oftmals über lange Zeit auch einzige – fachliche Begleitung für diese Patienten.
