Zusammenfassung
Die Projektionsradiographie des Beckens und der Lendenwirbelsäule ist die Methode der ersten Wahl in der primären Abklärung der Spondylarthritis (SpA). Die radiographische Sakroiliitis wird gemäß den modifizierten New-York-Kriterien graduiert. Um die diagnostische Latenzzeit der SpA zu verkürzen, sollte frühzeitig die Magnetresonanztomographie (MRT) in der Bildgebung eingesetzt werden. Die Sakroiliitis in der MRT ist durch das subchondrale Knochenmarködem an den Sakroiliakalgelenken (SIG) definiert und dem positiven HLA-B27 als Kriterium für die SpA gleichwertig. Bei unklaren Befunden an den SIG hilft die Abklärung der gesamten Wirbelsäule weiter, weil Manifestationen an der Brust- und Lendenwirbelsäule, Romanus- oder Andersson-Läsionen sowie entzündliche Veränderungen der Wirbelkörperpedikel oder der zygoapophysealen Gelenke und der Längsbänder charakteristisch für die SpA sind. Die Prävalenz der seronegativen SpA ist höher als bisher angenommen. Die Bildgebung kann einen bedeutenden Teil dazu beitragen, die betroffenen Patienten aus dem großen Pool der Rückenschmerzpatienten herauszufiltern, und somit irreversible Schäden bei frühzeitiger adäquater Therapie verhindern.
