Zusammenfassung
Lithiumaugmentation ist ein additives Therapieverfahren, das zur Behandlung von Patienten mit schwerer depressiver Episode eingesetzt wird, die auf einen Therapieversuch mit einem Antidepressivum nicht angesprochen haben. In dem Kapitel werden besondere Vorteile der Therapie (antisuizidale Wirkung, gute Wirkevidenz) und die überschaubare Zahl relativer sowie absoluter (Nierenversagen, Myokardinfakt) Kontraindikationen beleuchtet. Die praktischen Durchführungsschritte umfassen vor Therapiebeginn: Indikations- und Kontraindikationsprüfung, Voruntersuchungen (internistisch-neurologische Untersuchung, EKG, Schwangerschaftstest, Bestimmung von Körpergewicht, Natrium, Kalium, Kalzium, Kreatinin, TSH), Patientenaufklärung (enge therapeutische Breite, Flüssigkeitshaushalt, Nieren- und Schilddrüsenfunktion, Komedikation, Therapiemonitoring). Die Therapie wird mit Tagesdosen von 12,2–24,4 mmol begonnen und die Wirkung etwa 14 Tage nach Erreichen des Zielspiegels von 0,6–0,9 mmol/l evaluiert. Für typische Probleme im Rahmen der Therapie (Tremor, Durchfälle, Hypothyreose, Niereninsuffizienz) werden Bewältigungsstrategien besprochen.
