Zusammenfassung
Psoriasis vulgaris ist eine häufige HLA-assoziierte chronische, entzündliche Erkrankung der Haut und teilweise auch des Bindegewebs- und Stützapparats in Form der Psoriasis-Arthritis. Psoriasis entsteht auf einer komplexen genetischen Prädisposition und besitzt eine familiäre Häufung, die oft über Generationen nachweisbar ist. Jedoch tritt Psoriasis auch ohne familiären Hintergrund und bekannte genetische Assoziationen auf. Die Hautveränderungen der Psoriasis sind scharf begrenzte, silbrig-weiß schuppende erythematosquamöse Plaques in typischer Verteilung, die auf einer entzündlich gesteigerten Epidermopoese beruhen und weite Bereiche der Haut bedecken können. Neben den klassischen Hautmanifestationen als Guttata-Psoriasis oder Plaque-Psoriasis gibt es verschiedene generalisierte oder lokalisierte Sonderformen mit oder ohne Pusteln. Etwa 20 % der betroffenen Patienten entwickeln eine seronegative Polyarthritis. Gehäuft mit Psoriasis assoziierte Erkrankungen sind Morbus Crohn und Uveitis. In Abhängigkeit von Dauer und Schwere beinhaltet Psoriasis zudem ein eigenständiges Risiko für Übergewicht, metabolisches Syndrom, kardiovaskuläre Erkrankungen und eine gesteigerte Mortalität. Psoriasis wird heute als vornehmlich T-Zell-vermittelte Autoimmunerkrankung angesehen, bei der die Aktivierung eines bestimmten funktionellen Typs autoreaktiver T-Zellen gegen Hautgewebezellen die chronische Entzündung verursacht.
