Klinische Studien auf der Basis molekularer Charakterisierung von Tumoren

Zusammenfassung

Retrospektive und prospektive Untersuchungen belegen, dass die Machbarkeit von molekularem Profiling, d. h. der genomischen Charakterisierung von Tumoren, im akademischen und auch im gehobenen Routine-Setting gegeben ist. Die Frage, ob bzw. inwieweit eine auf Basis derartiger Analysen zusammengestellte („matched“) Therapie zu besseren Ergebnissen als Standardtherapie führt, muss als nach wie vor nicht endgültig geklärt eingestuft werden. Die Studien, die einen Vorteil für gematchte Therapie nahelegen, sind retrospektiven Charakters und damit für Bias anfällig. Die bisher einzigen beiden prospektiv randomisierten Studien, die sich dieser Fragestellung angenommen haben, verliefen negativ, und Ergebnisse weiterer prospektiv randomisierter Studien sind noch ausständig. Wenngleich einzelne, meist einarmige Strategiestudien einen Vorteil für Therapie gemäß molekularem Profiling suggerieren, ist die Datenlage bisher nicht reif genug, um ein derartiges Vorgehen oder sogar eine Kommerzialisierung derartiger Testungen jenseits von laufend anzupassenden Behandlungsrichtlinien außerhalb von klinischen Prüfungen, d. h. für die klinische Routine, zu empfehlen.
