Zusammenfassung

Krebserkrankungen während der Schwangerschaft sind selten. Geschätzt ist 1 von 1000 schwangeren Frauen betroffen. Die Betreuung von Krebserkrankungen in der Schwangerschaft muss interdisziplinär in einem Team bestehend aus Onkologen, Geburtshelfern, Pädiatern und Psychologen bestehen. Die geburtshilfliche Überwachung muss gemeinsam mit einem perinatologischen Zentrum mit der Möglichkeit einer Pränataldiagnostik entsprechend DEGUM II erfolgen. Die Therapie von Krebserkrankungen in der Schwangerschaft kann grundsätzlich analog zur Therapie außerhalb einer Schwangerschaft erfolgen, wobei Hormontherapien und zielgerichtete Therapien nicht möglich sind. Die bildgebende Diagnostik wird in Form von Ultraschalluntersuchungen, Mammografien, Röntgenuntersuchungen des Thorax und Kernspintomografien durchgeführt. Operationen können im Verlauf der Schwangerschaft grundsätzlich ohne Bedenken durchgeführt werden. Die Durchführung einer zytostatischen Chemotherapie ist bis zur 14. Schwangerschaftswoche (SSW) aufgrund des hohen Fehlbildungsrisikos nicht möglich. Ab der ca. 20. SSW sind bei Durchführung einer Chemotherapie keine fetalen Fehlbildungen zu erwarten. Die am häufigsten diagnostizierten Krebserkrankungen während der Schwangerschaft umfassen das Zervixkarzinom, das Mammakarzinom, das maligne Melanom gefolgt von Lymphomen und Leukämien.
