Zusammenfassung
Als zentrales Stoffwechselorgan ist die Leber für die Metabolisierung antineoplastischer Substanzen äußerst wichtig. Die Erhöhung der Leberwerte ALT, AST, AP und/oder Bilirubin werden als Anhaltspunkte genutzt, ob es bei der Gabe onkologischer Arzneimittel zur Lebertoxizität kommt. Leberschäden könnten sowohl an den Hepatozyten als auch an den Cholangiozyten stattfinden, letztere mit typischer Erhöhung von AP, Gamma-GT und Bilirubin. Selten, aber bedeutsam sind auch Lebervenenschäden, wie venookklusive Erkrankung (VOD), die zu schwerer Einschränkung der Leberfunktion führen. Als neue Toxizitätsform unter der Gabe von Immuncheckpointinhibitoren ist auch die immunvermittelte Hepatitis beschrieben worden, die bei 5–8 % der Patienten unter der oben genannten Therapie in schwererem Ausmaß auftritt. Nicht zuletzt ist eine bleibende Steatose der Leber leichter bis mittelgradiger Ausprägung nach einer stattgefundenen Chemotherapie ein häufig berichtetes Phänomen in der Nachsorge der Patienten.
