Therapeutische Endoskopie im Gastrointestinaltrakt

Zusammenfassung

Trotz stetig weiterentwickelter Operationsverfahren führen Anastomoseninsuffizienzen im Gastrointestinaltrakt nach wie vor zu einer relevanten Morbidität und Mortalität. Die Entwicklung der interventionellen Endoskopie hat ein konservativ-interventionelles Management dieser Komplikationen in vielen Fällen möglich gemacht. Bei Anastomoseninsuffizienzen im oberen Gastrointestinaltrakt ist die Stenttherapie in den vergangenen Jahren zum Standard geworden, im Rektum die endoskopische Vakuumtherapie. In letzter Zeit erweitern zwei neue Entwicklungen die Möglichkeiten des endoskopischen Komplikationsmanagements: die endoskopische Vakuumtherapie am oberen Gastrointestinaltrakt, die zunehmend in der Routine Anwendung findet, und der Einsatz des OTSC (Over-the-Scope-Clip) zum Verschluss von Fisteln und Leckagen im oberen und unteren Gastrointestinaltrakt.
