Hybrid Lengthening Techniques: Lengthening and Then Nailing (LATN) and Lengthening and Then Plating (LAP)

Abstract

Limb lengthening in skeletally mature patients has unique issues. Prolonged time in a frame confers several disadvantages. It is possible to use hybrid techniques that include both external and internal fixation such as intramedullary rods and plates for the consolidation stage. We believe that lengthening and then nailing (LATN) or plating (LAP) are safe and effective techniques for limb lengthening and deformity correction. LATN has superior stability and enhancement of bony healing over LAP, but osteotomy should be at least 10 cm distal to the knee joint. LAP can be used with shorter proximal bone segments when the osteotomy is in close proximity to a joint such as in a periarticular deformity with shortening.
