Zusammenfassung
Der Ausdruck Glomerulonephritis bezeichnet eine entzündliche Veränderung des Glomerulus. Die Einteilung und Beschreibung der Glomerulopathien erfolgt deskriptiv. Die meisten Glomerulonephritiden sind autoimmuner Genese und gehen meist mit einem nephritischen Syndrom einher. Im Vordergrund stehen Immunkomplexerkrankungen. Glomerulopathien mit einer isolierten Schrankenstörung des glomerulären Filters präsentieren sich mit einer isolierten Proteinurie bis hin zu einem nephrotischen Syndrom. Das nephritische Syndrom umfasst die Kardinalsymptome (Mikro-)Hämaturie, subnephrotische Proteinurie und Bluthochdruck. Das nephrotische Syndrom ist definiert als große Proteinurie mit Hypalbuminämie, Ausbildung von Ödemen und einer Hypercholesterinämie. Das klinische Syndrom der rapid-progressiven Glomerulonephritis (RPGN) beschreibt einen raschen Nierenfunktionsverlust über Tage bis Wochen bis hin zur terminalen Niereninsuffizienz. Eine chronisch progressive Niereninsuffizienz ist eine häufige Spätform zahlreicher glomerulärer Erkrankungen, bei der es über längere Zeiträume zu einem kontinuierlichen weiteren chronischen Nierenfunktionsverlust kommt.
