Periphere arterielle Verschlusskrankheit (pAVK): Revaskularisation, incl. Sekundärprophylaxe

Zusammenfassung

Die Diagnose der peripheren arteriellen Verschlusskrankheit (pAVK) wird in den letzten Jahren häufiger gestellt. Die Prävalenz der pAVK als Erkrankung des höheren Lebensalters steigt mit der demographischen Entwicklung. Es besteht ein breites Spektrum an klinischen Manifestationen: die akute Extremitätenischämie, die chronische kritische Ischämie inkl. Ruheschmerz und Nekrose / Gangrän, das Stadium der Claudicatio intermittens und die asymptomatische pAVK. Patienten mit einer symptomatischen pAVK leiden häufig an multilokulären Gefäßläsionen und besitzen internistische Komorbiditäten. Es bieten sich unterschiedliche therapeutische Möglichkeiten an, sodass für jeden Patienten eine bestmögliche Strategie unter Risiko-Nutzen-Abwägung erfolgen sollte. Die Indikation zu einer Revaskularisation ist abhängig von der klinischen Symptomatik, der Lokalisation und Läsionsausprägung der arteriellen Verschlüsse oder Stenosen und dem individuellen Behandlungswunsch des Patienten. Für die optimale Therapie ist primär eine Einschätzung des klinischen Stadiums der pAVK Voraussetzung. Insbesondere in der endovaskulären Kathetertherapie der pAVK sind enorme technische Fortschritte erreicht worden, die somit meist Vorrang vor einer konventionellen chirurgischen Bypasstherapie besitzt.
