Zusammenfassung
Karzinome des Magens gehören zusammen mit Karzinomen des ösophagogastralen Übergangs weltweit zu den häufigsten tumorbedingten Todesursachen. Mehr als 90 % der Tumoren des Magens sind Adenokarzinome, wobei histologisch diffuse und intestinale Typen unterschieden werden können. Das 5-Jahres-Überleben beträgt dabei über alle Stadien gemittelt ca. 30 %. Meistens sind zu Beginn der Erkrankung keine oder unspezifische Symptome vorhanden. Die Diagnose wird hauptsächlich endoskopisch gestellt. Therapieoptionen sind beim Magenfrühkarzinom die lokale endoskopische Resektion, beim lokal begrenzten Karzinom die chirurgische Resektion und bei lokal fortgeschrittenen Tumoren in Europa vor allem die perioperative Chemotherapie, seltener die präoperative Radiochemotherapie sowie die adjuvante Radiochemotherapie und in Ausnahmefällen die adjuvante Chemotherapie. Jedoch ergeben sich auch bei kurativ intendierter Operation in Stadien mit lokal fortgeschrittenen Tumoren häufig schlechte 5-Jahres-Überlebensraten. Im Rahmen der palliativen Chemotherapie sind in der Erstlinie 5-Fluorouracil und platinbasierte Regime etabliert.
