Zusammenfassung
Vorhofflattern gehört nach Vorhofflimmern zu den häufigsten Rhythmusstörungen des Erwachsenen. Zugrunde liegt eine Kreiserregung innerhalb des Vorhofmyokards. Am häufigsten kreist das Vorhofflattern um den Trikuspidalklappenanulus. Eine Gefährdung des Patienten kann aus einer schnellen Überleitung der atrialen Erregung auf die Herzkammern sowie durch Thrombusformation mit Gefahr von embolischen Ereignissen resultieren. Typische Symptome sind Herzrasen, Leistungseinschränkung und Dyspnoe. Es wird die häufige typische Form von der seltenen atypischen Form abgegrenzt. Typisches Vorhofflattern kann bereits im 12-Kanal-EKG mit hoher Sicherheit diagnostiziert werden. Die endgültige Diagnose wird letztlich in der elektrophysiologischen Untersuchung gestellt. Die Akuttherapie erfolgt analog der Akutbehandlung des Vorhofflimmerns. Als definitive Standardtherapie des typischen Vorhofflatterns gilt die Ablationsbehandlung des kavotrikuspidalen Isthmus mit geringem prozeduralem Risiko und hoher Erfolgsrate. Auch atypische Formen des Vorhofflatterns sind einer Verödungstherapie prinzipiell zugänglich mit allerdings häufig erhöhtem prozeduralem Aufwand und Risiko sowie reduzierten Langzeiterfolgsraten.
