Zusammenfassung
Der „hypertensive Notfall“ stellt definitionsgemäß eine akute Erhöhung des systolischen und diastolischen Blutdrucks mit gleichzeitig bluthochdruckbedingter Endorganschädigung dar. Eine akute Blutdruckerhöhung ohne Zeichen von Endorganschäden wird hingegen als „hypertensive Dringlichkeit“ bezeichnet. Die Mehrzahl der Patienten mit einer akuten Blutdruckentgleisung weisen eine „hypertensive Dringlichkeit“ auf, nur bei etwa 1/3 besteht ein „hypertensiver Notfall“. Im Gegensatz zur „hypertensiven Dringlichkeit“ liegt jedoch beim „hypertensiven Notfall“ eine lebensbedrohliche Situation vor, die eine sofortige Blutdrucksenkung erfordert, um den Fortgang hochdruckinduzierter Organschäden zu verhindern. Eine klinische Überwachung ist dabei zwingend. Möglicherweise werden in Zukunft die beiden Begriffe im Hinblick auf die akute Therapienotwendigkeit etwas revidiert Die sogenannte „maligne Hypertonie“ stellt klinisch und therapeutisch keine eigene separate Erkrankung mehr dar. Trotz allgemeiner Therapieempfehlungen sollte im Hinblick auf die Art des hypertensiven Notfalls und die Besonderheiten des einzelnen intensivmedizinischen Patienten das therapeutische Vorgehen individuell angepasst werden.
