Nichtinvasive Beatmung zur Therapie der akuten respiratorischen Insuffizienz

Zusammenfassung

Das respiratorische System besteht aus 2 weitgehend unabhängig voneinander arbeitenden Anteilen: dem Lungenparenchym für den Gasaustausch und der Atmungspumpe für die Ventilation. Schwerwiegende Erkrankungen führen zu unterschiedlichen Formen der akuten respiratorischen Insuffizienz (ARI): im Lungenparenchym kommt es zur hypoxämischen ARI, bei der Atmungspumpe zur hyperkapnischen ARI. Maschinelle Beatmung bessert den gestörten Gasaustausch und entlastet die Atemmuskulatur. Durch NIV lassen sich Komplikationen der invasiven Beatmung häufig vermeiden.

Wenn möglich, sollte die NIV bei Erkrankungen, die zur hyperkapnischen ARI führen, angewendet werden, da sie hier die Intubations- und Mortalitätsrate deutlich senkt (besonders für pH 7,2–7,35).

Der Stellenwert der NIV bei hypoxämischer ARI ist deutlich geringer. Für das kardial bedingte Lungenödem ist der Effekt von CPAP bzw. NIV in Kombination mit der medikamentösen Standardtherapie belegt. Hingegen kann die NIV bei der nichtkardialen hypoxämischen ARI nicht grundsätzlich als Therapie empfohlen werden.
