Zusammenfassung

Obgleich die Rheumatologie als Fachrichtung der Inneren Medizin in aller Regel eher als Disziplin gilt, in der Entscheidungen nach reiflicher Überlegung und langwieriger Diagnostik gefällt werden, besteht bei einzelnen Krankheitsmanifestationen unmittelbarer, auch intensivmedizinischer Handlungsbedarf. Die wichtigsten dieser rheumatologischen Notfälle werden im Folgenden kurz dargestellt:

  • akute Dislokation im Halswirbelsäulenbereich bei rheumatoider Arthritis (bedeutsam auch für Intubation/Beatmung!),

  • Arteriitis temporalis,

  • systemische Sklerose mit der möglichen Folge einer renale Krise,

  • katastrophales Antiphospholipidantikörpersyndrom (APS),

  • Goodpasture-Syndrom,

  • Lupuskrise.
