Zusammenfassung
Aufgrund mehrerer, z. T. auch multifaktorieller Pathomechanismen ist die Osteochondritis dissecans (OD) eine nicht klar definierte Erkrankung. Deshalb sollten diese Erkrankungen besser als osteochondrale Läsionen (OCL) bezeichnet werden. Eine osteochondrale Läsion des Ellenbogengelenks tritt meist im Bereich des Capitulum humeri auf. Betroffene sind meist Jugendliche, die intensiv Turnen, Wurf- und Überkopfsportarten ausüben. Die OCL des Ellenbogens wird vor allem in amerikanischen und japanischen Publikationen bei jungen Baseballspielern beschrieben. Sehr wahrscheinlich basiert in diesen Fällen die Erkrankung auf einer mikrotraumatischen Genese. Abzugrenzen von der OCL ist der Morbus Panner. Hierbei handelt es sich um eine juvenile Osteonekrose des gesamten Capitulum humeri, deren Äquivalent an der Hüfte der Morbus Perthes ist. Er tritt hauptsächlich bei Jungen unter 10 Jahren auf und hat einen benignen Krankheitsverlauf.
