Zusammenfassung

Abrissverletzungen der Epikondylen stellen die dritthäufigste Ellenbogenfraktur dar. Hauptsächlich ist der Epicondylus ulnaris (99 %) betroffen, nur selten der Epicondylus radialis oder die Kollateralbänder. Zwei Drittel aller Epicondylus-ulnaris-Frakturen treten im Rahmen einer Ellenbogenluxation auf. Da der Epicondylus nicht am Längenwachstum des Knochens beteiligt ist, existiert kein Korrekturpotenzial und es treten keine Wachstumsstörungen auf. Ziele der Therapie sind die Wiederherstellung der vollen Funktion und die Vermeidung einer Instabilität. Nicht oder wenig dislozierte Befunde werden konservativ in einer Oberarmgipsschiene behandelt. Die Indikation zur operativen Refixierung besteht bei einer kompletten Nervus-ulnaris-Läsion, bei intraartikulär eingeschlagenem Epicondylus-ulnaris-Fragment, bei deutlich dislozierten Frakturen oder bei Mobilitätswunsch des Patienten. Die Zugschraubenosteosynthese ermöglicht eine frühfunktionelle Mobilisation, die K-Draht-Osteosynthese ist bei kleinen Patienten oder kleinen Fragmenten indiziert. Keine Therapieform verhindert eine Pseudarthrosebildung gänzlich, bei korrekter Operationstechnik sind die Ergebnisse jedoch sehr gut.
