Zusammenfassung
Schwere Verbrennungsverletzungen mit Polytrauma sind selten, stellen ein komplexes Behandlungsproblem dar und bedingen die Behandlung in einer Klinik der Maximalversorgung, die sowohl ein Verbrennungszentrum als auch eine Unfallchirurgie vorhält. Die häufigsten Verletzungsursachen sind Verkehrs-, Strom-, Arbeitsunfälle sowie Explosionen. Das Management eines Schwerverbrandverletzten mit Polytrauma ist mit dem eines polytraumatisierten Patienten ohne thermische Verletzung vergleichbar. Die traumatische Gesamtbelastung sollte zügig erkannt und ein verletzungsadaptierter Behandlungsplan nach dem Damage-Control-Prinzip erstellt werden. Eine enge interdisziplinäre Zusammenarbeit ist Voraussetzung und erfordert höchste Aufmerksamkeit und Kooperation aller Beteiligten. Insbesondere der zeitliche Ablauf der notwendigen Schritte bietet Diskussionspotenzial zwischen den Fachabteilungen und muss angesichts der konkreten Verletzungsmuster stets im Sinne einer individualisierten Medizin entschieden werden.
