Zusammenfassung
Der therapeutische Schlafentzug gehört zum Armamentarium der Behandlung depressiver Erkrankungen und wird in den meisten deutschsprachigen Kliniken durchgeführt. Während der totale und der partielle Schlafentzug (der 2. Nachthälfte) sowohl unter stationären als auch unter ambulanten Bedingungen gut durchgeführt werden kann, sind die Phase-Advance-Therapie und der REM-Schlafentzug wegen des hohen Personalaufwands bzw. der Notwendigkeit einer apparativen Ausstattung vorwiegend den Forschungsabteilungen vorbehalten. Das Indikationsspektrum schließt sowohl uni- als auch bipolare Depressionen in der akuten Phase der Erkrankung mit ein, es können jedoch auch chronifizierte oder therapieresistente Verlaufsstadien behandelt werden. Unter den spärlich vorhandenen Prädiktoren lassen klinische Erfahrungen einen positiven Behandlungseffekt bei Vorliegen von depressiven Vitalstörungen sowie bei typischen Tagesschwankungen mit einer Befindlichkeitsverschlechterung am Morgen und einer Besserung in den Nachmittags-/Abendstunden erkennen. Die Medikation sollte während des therapeutischen Schlafentzugs möglichst konstant gehalten werden, jedoch sedierende Medikamente sollten nicht verabreicht werden. Vereinzelte Studien lassen erkennen, dass der therapeutische Effekt des Schlafentzugs durch die am Morgen nach dem Schlafentzug angewandte Lichttherapie aufrechterhalten werden kann.
