Zusammenfassung
Narbige Verengungen der Harnröhre infolge Traumata oder Entzündungen führen zu Harnröhrenstrikturen. Neben Beschwerden und Schmerzen beim Wasserlassen können diese in späten Stadien zu Niereninsuffizienz, selten auch zu Urosepsis führen. Diagnostisch kommen Harnstrahlmessung, Sonografie, Urinkultur, retrograde Urethrografie und MCU zum Einsatz. Bougierung und Einlage von Stents gehen mit recht hohen Rezidivraten einher; lange Zeit galt die Urethrotomia interna nach Sachse als „Goldstandard“, in jüngerer Zeit wird hier ein Holmiumlaser genutzt. Die geringsten Rezidivraten zeigen die Exzision und Anastomose bei kurzen Strikturen und sonst verschiedene rekonstruktive Operationen unter Verwendung ganz unterschiedlicher Ersatzgewebe.
