Zusammenfassung
Diffuse ZNS-Funktionsstörungen, die nach akuter oder chronischer Leberfunktionsstörung auftreten und klinisch, neuro-psychologisch oder mittels neurophysiologischer Verfahren nachweisbar sind, werden nach Ausschluss anderer Ursachen als hepatische Enzephalopathie gedeutet. Je nach zugrunde liegender Pathologie und Verlaufsform werden drei Typen (A-C) und vier Stadien (I-IV) unterschieden. Die hepatische Enzephalopathie geht oft, aber nicht zwingend, mit einer Hyperammonämie einher. Im EEG sind Rhythmisierungen wie triphasische Wellen typisch. Die Bildgebung kann bis auf ein Hirnödem (v.a. im akuten Leberversagen) ohne wegweisenden zerebralen Befund bleiben. Wichtig ist der Ausschluss anderer Ursachen von Bewusstseinsstörungen wie fokale Anfälle (EEG), Hypoglykämien und zerebrale Blutungskomplikationen (CCT) durch Koagulopathien. Krampfanfälle, Tremor und Asterixis sind häufig im zerebralen Allgemeinsyndrom dieser Enzephalopathie anzutreffen, aber auch Zirrhose-assoziierte Parkinson-Syndrome und durch die Komorbiditäten toxisch ausgelöste Schäden (PNP, Ataxie). Therapeutisch wird im akuten Leberversagen neben den konservativen Verfahren zur Ammoniakverminderung (Laktulose usw.) gegenwärtig die Hämofiltration und die Hypothermiebehandlung diskutiert, um Zeit bis zur lebensrettenden Organtransplantation zu gewinnen.
