Neglected Posterior Dislocations and Treatment Modalities

Abstract

Posterior dislocation of the shoulder is a rare injury and often untreated due to missed diagnosis. To prevent persistent posterior shoulder dislocation (PSD), a meticulous examination and three (ap, axial, Y-view) radiographs are crucial, although patients often present delayed in time just with minor limitation concerning range of motion. PSDs usually come along with anteromedial lesions of the humeral head (reverse Hill-Sachs lesion) that can lead to engaging and locking at the glenoid rim. In neglected PSDs the size of the humeral defect and the time interval between injury and diagnosis are the leading parameters regarding the treatment procedures and the postoperative outcome. Anatomic reconstruction or combined anatomic and nonanatomic procedures allow better results than nonanatomic procedures.
