Zusammenfassung

Depressive Verstimmungen und Episoden sind Endstrecke verschiedenster genetischer, biografischer, lerngeschichtlicher, konflikthafter und gelegentlich auch körperlicher Prozesse. Depressive Episoden haben einen vergleichsweise typischen Verlauf und sind psychotherapeutisch gut behandelbar. Psychotherapie gilt heute bei den besonders häufigen leichtgradigen Episoden als Mittel der Wahl, da die medikamentöse Behandlung in diesem Bereich eine ungünstige Kosten-Nutzen-Beziehung zeigt (Nationale Versorgungsleitlinie Unipolare Depression; Härter et al. 2011; ▶ Abschn. 11.3). Auch im Bereich der mittelschweren und der schweren Depressionen kann inzwischen davon ausgegangen werden, dass, nicht zuletzt aufgrund der größeren Nachhaltigkeit und der besseren „Compliance“, Psychotherapie ein unabdingbarer Bestandteil der Therapie ist (NICE 2009). Dies hat v. a. auch dadurch seine Berechtigung, dass Persönlichkeitsbeeinträchtigungen und Beziehungsstörungen bzw. das bekanntermaßen häufige Vorliegen komorbider Störungen wie Angst, Sucht, Schmerz, Essstörungen etc. die Indikation für eine Psychotherapie meist zwingend erscheinen lassen.
