Transcranial Doppler Ultrasonography in Emergency and Intensive Care

Abstract

Transcranial Doppler ultrasound allows measurements of blood flow velocity to be made from the basal intracerebral vessel. The major advantages of transcranial Doppler ultrasound are that it is noninvasive, relatively cheap, can be performed with portable machines, allows monitoring for prolonged periods, and has a high temporal resolution making it ideal for studying dynamic cerebrovascular responses. Transcranial Doppler ultrasound can be used to detect vessel narrowing from a variety of causes, including vasospasm, and also can be used to detect cerebral emboli and to evaluate CO2 reactivity, autoregulation, and the response to certain medications, as well as to indicate progressive obstruction of the cerebral circulation as seen in conditions leading to brain death. It is also useful to guide clinical management and therapeutic choices.
