The Pharmacist Guide to Implementing Pharmaceutical Care

Abstract

Pharmaceutical care in the US and Canada is regulated at the level of the state/province generating differences in the practice of pharmacy across the countries. Pharmacist counseling and utilization review are generally required to be offered at dispensing in community pharmacies. There is a trend toward the development of pharmaceutical care services provided at the community pharmacy , the integration of the pharmacist in the healthcare team, and the implementation of collaborative agreements expanding the role of the pharmacist in patient care. This chapter is divided into two sections, one for the United States of America, and one for Canada.
