Abstract

Complex anatomy is undoubtedly one of the main obstacles to cleaning and disinfection of the root canal system by irrigants. The diameter of the areas that need to be reached in combination with the viscosity of the irrigants seems to limit irrigant flow mostly within the wider areas irrespective of the method used; so, transport of molecules/ions to narrow areas relies primarily on diffusion, a much slower and inefficient process. Syringe irrigation performs similarly to other irrigation methods in the main root canal, provided that the needle is inserted close to working length and the canal is adequately enlarged. However, additional delivery and activation methods are needed to clean fins, uninstrumented oval extensions, isthmuses, and accessory canals. Adding more impediments to this challenging procedure by reducing the size of the access cavity or that of the main root canal should be avoided.
