Abstract

This chapter will provide an overview of the structural aspects and processes that characterize healthcare transition (HCT) models. Comparisons and contrasts between HCT models that focus on the transfer of care and those that are more comprehensive in scope will be reviewed. Outcomes associated with the measurement of HCT service and adolescents and young adults with special healthcare needs will be discussed. Points of emphasis for clinical application are featured in the chapter tables and case example.
