Abstract
This chapter focuses on this emerging topic, highlighting the hospitalist’s perspective on transitioning AYASHCN into adult models of inpatient care. Using a variety of clinical scenarios, common issues facing inpatient providers and delineation of critical elements of transition planning for these youth, their families, and healthcare teams are addressed. An argument is made for the imperative of integrating hospital care into a comprehensive approach to transition planning, particularly for children with more complex medical needs and anticipated ongoing hospitalizations. Both pediatric and adult hospitalists, as well as other inpatient providers, could adopt strategies to effectively plan and carry out this element of the transition process. Long-term proactive strategies and short-term fixes are proposed.
